FAQs
Do you take my insurance?
I am officially in-network with Aetna, Blue Cross Blue Shield, and United Healthcare. It is the responsibility of the patient to call and check nutrition counseling/therapy benefits prior to initial appointment; to do this, please use the information below to verify your coverage. If the representative asks for a preventative code, please provide them with the following: Z71.3 (this is a general nutrition counseling code). You may need a medical diagnosis for coverage; if you do please obtain one from your medical provider or use some of the following example ICD-10 Diagnosis Codes below:
E66.3- overweight, E66.9- obesity, E66.01- morbid obesity
E11.65- diabetes
E78.5- hyperlipidemia/high cholesterol
You will want to have the following info handy when you make the call:

Date of Birth (say the age if you are calling about a minor)
Member ID (from your insurance card)
Provider Name: NutrishOn LLC
Provider Tax ID: 873242411
Provider NPI: 1316608276

What to Ask Your Insurer
1. Do I have nutritional counseling benefits with the following CPT and Diagnosis codes:
If out-of-network, at what rate will I be reimbursed for services?
2. What is my responsibility (deductible, copay, coinsurance):
If deductible applies, how much has been met?
3. Do I have any nutritional counseling benefits covered under the preventative care portion of my plan:
4. Is there a visit limit (for example, 7 hours per year) or a unit per visit limit (for example, 4 units per visit)
5. Is telehealth covered? If so, is my responsibility the same:
Is there an expiration date for telehealth:
6. Do I need a referral?
7. Is a prior-authorization required? If so, what is the process?
8. Can I have your name and a reference number?
Document the date/time of the call for future reference.
How do appointments work?
All appointments are held virtually- this means they are held online. I use a platform called Kalix to host our appointments to maintain HIPAA compliance to protect your personal health information. During initial appointments, together we will review your intake document to make sure I have “the full picture” when it comes to your health and wellness; from there, we will develop goals to work towards and/or discuss meal plans. During follow-up appointments, we discuss your progress and any changes that need to be made to help you achieve your goals.
How do payments work?
I currently accept credit/debit/bank transfer payments through Square. After an appointment, expect to receive a bill from me through Square either through your email or phone.
“I’m looking to lose weight…how much can I expect to lose?”
Weight loss will be different for everyone depending on your goals and your efforts towards them; working with me you should expect to lose between 0.5-1.5 lbs per week following my recommendations.
Why don’t you have a phone number listed for scheduling?
Using email/contact through Kalix is my preferred method of contact because I am able to respond to you faster and more easily between appointments and activities throughout my day.
